	The Arizona Football Officials Association Sportsman Form
The AzFOA has a long-standing tradition of recognizing that school that has embodied the highest caliber of sportsmanship consistently displayed through out the football season. To enhance program credibility and fairness, all Crew Chiefs are encouraged to fill out the following evaluation form for each contest that they work throgh out the regular season.
